Font of choice
OK - open the template in PSP, rotate all layers 90 degrees left
Select the large rectangle, float, defloat, resize Paper2 and paste as a new layer
selections, invert delete, select none, drop shadow 0 0 60 5
delete the original layer
Do the same on the next layer using Paper1
merge down the three frame layers so the are all in one layer
click inside the 3 frames with magic wand and floodfill with a pink gradient
paste your tube as a new layer, position, invert and delete
on the tube layer apply blinds 2 30 both boxes ticked and use a dark pink colour
add a drop shadow to the frame layer
lower the opacity of your tube slightly
decorate your tag using elements from the kit, sharpen & drop shadow all elements
0 0 70 5
Add your name to the tag
hide white background layer and merge visible all other layers
duplicate and on the bottomlayer adjust blur, gaussian blur 10
effects, texture effects mosaic glass

Add your copright information
resize & crop to your liking
delete background layer & save as a Png!
